There are a couple ways to request a license from The Lake Companies for Doc-Trak, Shop-Trak or Fact-Trak.
1. If you are a customer of ours, you can log into your OnTrak account using this link: OnTrak
a. Once you've logged on, go to the link that says License Request, like below.
b. Click on the product you'd like to request the license for and follow the questionnaire.
2. If you are a business partner or affiliate and need to request a license on behalf of one of your customers, you can use one of the links below to request a license.
If there is contact information you are unsure of, it is ok to put in 'NA'.

You should receive a license within 1-2 business days after requesting a license. If there are any questions or clarifications needed based on the license request information, someone will reach out to you.
